How to create a form with the Microsoft 365 Copilot app

1. In the Microsoft 365 Copilot app select Create from the navigation on the left side of the window.

2. Select Create a form.

3. Select one of the form templates and fill in the blanks. The more details you give Copilot the better your results are likely to be. For example:

   Create an office facility request form for equipment for event space. The form should gather details on requester info, event specifics, and equipment needed.

4. Select Add a file to base your form on information in Word, PowerPoint, or Excel files.

Copilot will create your form. You can add additional questions or edit any of the existing ones, and when you're ready send the form out to collect responses.
